Benefits of Case Management to Your Loved Ones

Sometimes all it takes is a helping hand; let SOURCE Care Management (SCM) be there to help you and/or your loved one(s) remain at home and in the community. There are many benefits of case management, the primary one being the improvement of quality of life, healthcare and overall health outcomes.

Our case managers work one on one with our members to coordinate their care in an effort to preserve and enhance their quality of life. All SCM members receive a needs assessment on admission by our well trained case managers. This assessment assists case managers and the SCM multi-disciplinary team to determine what services our members truly need in order to be able to remain safely in the home and community setting. Community services available to our members include: comprehensive primary medical care with a SCM participating physician; personal support services; emergency response systems; transportation to and from medical or medically related appointments; adult day health; home delivered meals; home health care and respite care. We can also coordinate alternative living residences for our members if necessary as well as help them to get durable medical equipment their physician may have ordered for them. SCM takes our responsibility to assist our members in finding resources not reimbursable by Medicaid very seriously. We persistently explore community and other resources to help meet our members’ needs. We also work with the Community Health Foundation to meet the extenuating needs of some of our members.
