Organization Information
SOURCE Care Management, LLC doing business as SOURCE and is a wholly owned subsidiary of Home and Community Services, Inc. (HCS). SOURCE provides a central, efficient, and cost-effective manner in which to supply clients and patients with needed home care and related services as a member of Home and Community Services, Inc (HCS).
Each SOURCE Care Management Office is led by an Administrator, under whose leadership, a multidisciplinary case management team provides primary care enhanced case management services.
HCS is a Georgia nonprofit corporation organized for charitable purposes within the meaning of Section 501(c)(3) of the Internal Revenue Code of 1986, as amended (the “Code”). HCS is a member in an integrated healthcare delivery system.
